Every strain of tubercle bacilli produces mutants at a specified frequency that are spontaneously resistant to any given drug.  If monotherapy is given, the large number of bacilli susceptible to that drug are killed while the few resistant mutants multiply and progressively achieve numbers that will ultimately make them to become the dominant strain.
