Directly observed therapy ensures that the medications are taken as prescribed, thus preventing irregularity and sub-inhibitory concentrations.  In this setting in Texas, directly observed therapy led to a significant reduction in the frequency of multidrug-resistant (isoniazid plus rifampicin resistance) relapse and acquired drug resistance.  As expected, primary resistance was not affected.
